Definition and Methodology
This indicator counts the total number of formal coordination structures and tools (e.g., referral protocols, joint case management procedures, data-sharing agreements, or other coordination mechanisms) that are developed, adopted, and actively used to ensure smoother transitions (continuity of care and supervision when individuals move from prison to probation, support to individualised rehabitation plans, etc.) and better cooperation between prison and probation services. Ensures continuity of care and supervision when individuals move from prison to community-based probation.
New mechanisms refer to coordination tools or agreements introduced for the first time.
Improved mechanisms refer to existing coordination tools or agreements that have undergone significant revisions or upgrades to increase their effectiveness, scope, or operational quality.
